1. To undertake timely assessments of need and to develop support plans relating to the outcomes of the assessment and to facliate hospital discharge.
2. To undertake safeguarding investigations and all parts of the safeguarding process as required by the service.
3. To carry a caseload of varied range and complexity.
4. To undertake Mental Capacity Act Assessment in accordance with the Mental Capacity Act 2005.

7. To provide advice and information to colleagues within the immediate team and to a wider range of stakeholders on the role of the social worker and the role of the service.
8. To work in line with the council and services policies and procedures including health and safety, data and case recording, supervision, data protection and information governance.
9. To participate in an urgent response system/ duty system as directed by the manager.
10. To participate in service developments.
11. To ensure that professional registration and DBS are current and up to date. To inform management if this is not the case.
12. To be responsible for your own CPD and participate in training, reflective practice and team meetings as require by the service.

13. To ensure that the manager is kept informed of any significant events relating to the service
14. To participate in audits and practice evaluations when required
15. To undertake best interest and practice teacher training as appropriate as part of CPD and grade progression.
16. To undertake Trusted Assessor Training in order to be able to prescribe simple equipment and assistive technology solutions in order to maximise an individual’s independence and ability to stay in their own home.
17 With the consent of the customer, to adopt an inter-disciplinary approach and work in full collaboration with partners in order to achieve best outcomes.
18. To undertake carer’s assessments and carer’s support planning.

Qualifications:
Essential: Social Work Qualification; BA in Social Work, CQSW
• Registerd with the Health Care Professions Council (HCPC)
• DBS clearence
You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be HCPC registered. Additionally, you must have at least one year post qualifying experience and 6 months' experience working in a hospital team or multi-disciplinary setting.

JOB PURPOSE:
As a social worker the post holder will be part of a multi-disciplinary team which provides health and social care to the local community bringing social work expertise and knowledge including the social model of disability to the service.
1. To carry out all duties within the principles of Personalisation and enable people to retain their independence for as long as possible
2. To actively identify and maximise opportunities to reable customers to ensure they continue as long as possible to live independently and maximise their choice and control by achieving best outcomes
3. To facilite safe and timely discharges from an acute setting and manage cases effectively and respond to customers’ needs in a timely manner within a fast moving and highly pressured environment
4. To appropirately offer and document advice given to customers and their carers/ families including, information and signposting, regarding statutory and voluntary resources to customers using the service
5. Where appropriate act as care co-ordinator to ensure a seamless service to customers throughout their time in the service.
6. To ensure that assessment and support planning is undertaken in line with the Care Act 2014, in a timely manner and according to identified risk factors and ensuring that where appropriate the individual budget allocations are commensurate to the assessed need and that the appropriate approval process is followed.
7. To take responsibility for the promoting and safeguarding the wellbeing of adults (and children) in your care and those who you come into contact with.
