JOB TITLE: RESIDENTIAL CASE MANAGER
Job Summary:
Locates, coordinates and monitors a variety of services and support functions that meet the needs of residents of Sanctum House.
Essential Duties and Responsibilities:

Services:
* Ensures residents receive, review and sign Intake and Discharge paperwork, including an Explanation of Rights, Responsibilities and Complaints.
* Coordinates the set-up of Medicaid and other entitlements.
*Using a person centered approach, coordinates treatment needs: therapeutic, medical, psychosocial, substance abuse, legal issues.
* Monitors individual/group behavioral interventions, providing assessment and feedback for residents and staff.
* Monitors residents’ participation in treatment programs outside and outside of the residential setting, and obtains supporting documentation.
* Facilitates skill-building groups designed for residents.
* Trains and supports staff to ensure their ability to conduct groups and facilitate other program components.
* Facilitates and participates in resident case reviews.
* Writes caseload reports; documents case management notes.
* Communicates with family and support services regarding resident’s progress and response to treatment.
* When appropriate acts as liaison between resident’s family and referring agency.
* Coordinates resident’s visitations and therapy sessions.
* Communicates with and provides referring agencies with resident’s progress reports.
* When applicable, attends court hearings and provides testimony concerning resident progress and response to residential care and treatment.
* Coordinates with therapists to ensure attendance at scheduled sessions and/or regarding issues as they arise, treatment planning, and that supporting documentation is completed.
* When appropriate, coordinates medication management with residential psychiatrist.
* Coordinates education services for residents, including procurement of transcripts and education plans, enrollment, etc.
* Other services as assigned or needed.
Supports:
* Assists therapists with development, implementation and monitoring of individual treatment plans.
* Participates in staff meetings.
* Functions as a Team Leader to maintain program structure and integrity.
* Attends and participates in all clinical meetings, including weekly supervision meetings.
* Ensures provision of clothing for residents as needed.
* Other support functions as assigned or needed.
Other:
* Develops and maintains positive relationship with stake holders, families, residents and employees.
*Work Hours- 8:00AM-4:30PM Monday – Friday. Available for additional hours and flexible scheduling, as needed.
*Reports to Executive Director
*Performs other duties as assigned.

Team Leader Responsibilities:
Team Leader will provide clinical oversight of clinical and non-clinical staff.

Education and Experience:
The ideal candidate will have experience working with victims of human trafficking , trauma and substance abuse. Master Degree in social work, psychology, counseling or a related area of study from an accredited school is preferred. Minimal acceptable credentialing is Bachelor’s Degree in the above disciplines. The ideal candidate will have: three years of experience in case management in a residential treatment facility; possess current Michigan certification, licensure and/or registration in areas of expertise; possess a valid driver license.


Language Skills:
The candidate is expected to demonstrate excellent written and verbal skills as well as the following skills: proficiency with the English language; ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations; ability to write reports, business correspondence, and procedure manuals; ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Reasoning Ability:
The candidate will be able to identify problems, collect data, establish facts, and draw valid conclusions; interpret an extensive variety of technical instructions in mathematical or diagram form; and understand and process abstract and concrete variables.

Work Environment:
This position may require local travel. The Case Manager will be required to drive an automobile and take clients to various appointments, recreational activities, etc. The Case Manager is responsible for maintaining decorum within the Home in accordance with the Guide for Sanctum House Residents and the Guide for Sanctum House Staff.
